Misheberach Form

This form is for CBI members to fill out on behalf of anyone, including themselves, who would like to be added to the list of names read during the Misheberach, or prayer for healing, during Kabbalat Shabbat and Shabbat morning services.

Names will remain on the CBI Misheberach list for one month upon submission, after which we ask that you resubmit names to stay on the list.
